"Akhtar has some floating bones in his knees which require keyhole surgery," Pakistan coach Intikhab Alam told The Associated Press.
The 33-year-old Akhtar made an unimpressive return to international cricket earlier this month when he was dropped after taking just one wicket for a total of 88 runs in two limited-overs games against Sri Lanka.
Alam said Akhtar will soon undergo surgery "after which he requires at least two to three weeks of rest."
Sri Lanka, which defeated Pakistan 2-1 in a one-day series after a crushing 234-run victory, is currently trailing 1-0 in the five-match limited-overs home series against India.
The team is due to arrive in Pakistan in the third week of February to play tests in Karachi and Lahore. Akhtar has not played test cricket since December 2007 against India. He was included in Pakistan's squad which beat the West Indies 3-0 in a one-day series in the United Arab Emirates in November, but didn't play a game after being injured in practice.
